How to join the Practice:-
We need
1. All your details. Name, address, Phone No, Mobile No, Email
2. Your medical history; conditions, medications, operations etc
3. A completed DPAS registration form
4. A direct debit mandate signed by the bank account holder
5. Your registration fee.
All this normally takes less than 10 minutes, and the paperwork is
sent in a prepaid envelope to our admin company.
You can then make an appointment.
If you wish, you can receive text and email reminders. These are
sent 3 days before your appointments.
The insurance is active once the form is complete.
